Alprazolam — best known by the US brand name Xanax — is a short-acting benzodiazepine used in some countries to treat severe anxiety and panic disorder. It is one of the most widely searched-for medicines online, but the situation in the UK is very different from the United States.

What is alprazolam?
Alprazolam is a benzodiazepine — the same family as diazepam, lorazepam and clonazepam. It works on GABA-A receptors in the brain to produce a calming, sedating, muscle-relaxant and anti-anxiety effect.
It is short-acting, meaning effects come on quickly and wear off relatively quickly. This makes it appealing for panic attacks, but also gives it a higher dependence and rebound-anxiety profile than longer-acting benzodiazepines such as diazepam.
Is alprazolam (Xanax) prescribed in the UK?
No — not as a routine UK medicine. Alprazolam is unlicensed in the UK. That means it does not have a UK marketing authorisation from the Medicines and Healthcare products Regulatory Agency (MHRA).
- NHS prescribers do not normally prescribe it.
- NICE guidance (CG113) does not recommend it for anxiety.
- It can only be supplied in the UK on a private, named-patient unlicensed basis — which is extremely rare and limited to specialist situations.
- It is a Schedule 4 (Part I) controlled drug under the Misuse of Drugs Regulations 2001.
Reputable UK online pharmacies — including RestWell — do not supply alprazolam. Any UK-facing site that offers it should be treated with extreme caution.
The "Xanax online" problem
Because alprazolam is unlicensed in the UK, almost all "Xanax" sold online to UK customers is counterfeit, illegally imported, or diverted from other countries. The MHRA, NCA, and UK police have repeatedly warned the public about:
- Tablets sold as Xanax that contain unknown amounts of alprazolam, or other benzodiazepines such as etizolam or flualprazolam — sometimes at lethal doses.
- Tablets that contain no alprazolam at all, or contain other unrelated and dangerous substances.
- Coroner reports linking counterfeit alprazolam to UK deaths, particularly in young people.
Buying alprazolam online from non-UK or unregulated sites is a criminal offence and a serious safety risk.
What is the UK equivalent of Xanax?
There is no direct one-to-one substitute. UK clinicians treat anxiety using evidence-based pathways recommended by NICE:
First-line: non-medicine treatment
- Cognitive behavioural therapy (CBT) — including online and group programmes available free on the NHS via Talking Therapies (you can self-refer).
- Self-help and stress management — breathing techniques, sleep, exercise, alcohol and caffeine reduction.
- Talking therapies for anxiety and panic disorder.
First-line medication (where appropriate)
- SSRIs such as sertraline, escitalopram or paroxetine.
- SNRIs such as venlafaxine or duloxetine.
- Pregabalin in some cases of generalised anxiety disorder.
These take 2–6 weeks to work but, unlike benzodiazepines, do not cause physical dependence in the same way.
Where a benzodiazepine is unavoidable
If a benzodiazepine is genuinely needed for very short-term, severe anxiety, the usual UK choice is diazepam — for the shortest possible course (usually 2–4 weeks) and only with a clear stopping plan, because of dependence risk.
Risks of benzodiazepines in general
- Dependence — even after a few weeks of regular use.
- Withdrawal seizures if stopped suddenly after regular use.
- Rebound anxiety and insomnia when wearing off — particularly marked with short-acting drugs like alprazolam.
- Sedation, memory impairment, falls.
- Fatal interactions with opioids, alcohol or other sedatives — a major cause of UK drug deaths.

If you are already taking alprazolam — whether on a private prescription or from another source — do not stop suddenly. Abrupt withdrawal from benzodiazepines can cause severe rebound anxiety, panic, insomnia, and in some cases seizures.
Speak to your GP. The standard UK approach is a structured taper, often by switching to an equivalent dose of diazepam (which is longer-acting) and reducing slowly over weeks to months. Your GP may also refer you to a specialist drug service if needed.
